Patient Referrals We will make every effort to see all new patients within 24-48 hours of referral. We need the following information at least 24 hours PRIOR to the patient's appointment/test:
- Patient's Name
- Date of Birth
- Social Security Number
- Patient's Weight
- Actual written Diagnosis and Order(s) (may be faxed to our office)
- Copy of Patient's Insurance card(s)
- Referral (if required)
This information is necessary for pre-certification and to verify eligibility prior to the patient's appointment.
| EMR (Electronic Medical Records)
Our office is going paperless with the implementation of our electronic medical records - Misys EMR. Office notes and reports may be e-faxed within 24 hours of the patient's appointment/test. Prescriptions are now filled with e-Prescribe.
